Description and Requirements

Position Summary: The Team Lead Care Management is responsible for hands-on leadership of a specific care team (which may include RN’s, Social Workers, and Coordinators) in order to provide direct oversight to clinical staff. This is accomplished through the active involvement and leadership of the care team's day-to-day operations and ensuring that staff is compliant with Healthfirst’s policies and procedures and act in accordance with contractual, state and federal guidelines. The Team Lead may serve as a medical resource to members, providers, and other care management team members. The Team Lead is assigned to a specific product line such as FIDA, CompleteCare, SNP, Medicaid, Medicare, PHSP, LTSS, etc.
Duties and Responsibilities: Supervises, analyzes, and coordinates care management daily activities to ensure that members receive the support and services they need, that departmental and regulatory goals and requirements are met.  Foster an atmosphere of open communication, collaboration, accountability, and empowerment to make decisions. Responsible for reviewing and managing staff performance. Supports continuous improvement and quality initiatives that improve Care Management processes across departments. Ensures adherence to stated goals of the Care Management Department and the overall organization. Oversees continuation, coordination, and delivery of individualized care plans and authorized services to members. Communicates and efficiently manages changes that impact employees and the team. Keeps current with trends and best practices as related to Care Management and Population Health to drive improvement of outcomes. Contributes to the development of member-centric care and initiates the process in accordance with established guidelines. Participates in special projects, including, coordinating regulatory, quality, and accreditation activities. Additional duties as assigned.
