Description and Requirements

The Field Care Coordinator (FCC) is responsible for conducting local home visits for Healthfirst members. During these home visits, the FCC may perform a home environment assessment, administer health questionnaires, retrieve necessary documentation, and/or facilitate a video visit with a Healthfirst Care Manager to conduct other required assessments. When needed, the FCC also supports Healthfirst clinical teams by providing translation services during field home visits.
Duties/Responsibilities: Schedules and conducts multiple home visits per day (clustered in small geographic areas) Supports the Healthfirst clinical team members, including UAS nurses and Care Managers, by interpreting the employee-to-member and member-to-employee conversation. Facilitates cross-cultural communication within allotted time for employee and member. Responds to and, when needed, escalates member questions, as directed Properly documents findings in Healthfirst information systems Collects documents from members, including health care proxys, agreements and other important member documents, and ensures they are routed to the appropriate Healthfirst recipient Additional duties as assigned. Minimum Qualifications: Bilingual in English and one or more of the following languages: Spanish, Mandarin, Cantonese High school diploma or GED Experience using health information systems to document or retrieve information Ability to travel around downstate New York which includes the 5 boroughs, Long Island, Orange County, and Westchester. In order to access data sources like the Uniform Assessment System (UAS), an unexpired NYS driver's license or NYS ID is required. Preferred Qualifications: Access to a vehicle and/or public transportation to travel around a designated and assigned area of downstate NY Work experience as an Interpreter/Translator or in a bilingual customer service role in the field and in a call-center Experience with home or field visits a plus Good computer skills, include solid understanding of Microsoft Outlook, Word and Excel. Experience with geriatric or other frail populations, including their caregivers, or in a health care office Familiar with areas of NYC, Long Island, and/or lower Westchester Compliance & Regulatory Responsibilities: Ensures member documentation is properly routed and maintains privacy of member PHI in accordance with HIPAA
Hiring Range*: Greater New York City Area (NY, NJ, CT residents): $46,200 - $62,400 All Other Locations (within approved locations): $40,100 - $58,240 As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
